Download and complete the appropriate lab slip for your case. Choose from our standard Dental Lab Slip or Ortho Lab Slip below.

Email Digital Scans & Completed Slip
Email your digital scans (.stl files / impression data) along with your completed lab slip directly to our production division.

We Will Manufacture Your Appliance
Our expert technicians manufacture your appliance to exact specifications with strict quality control. Standard laboratory turnaround is approximately 5–7 business days.

Completed Case Shipped via UPS
Your completed case will be safely packaged and shipped via UPS. We will provide the tracking number via email as soon as your case ships.
